Qualifications:
Licensed Registered Nurse with current valid license to practice in Virginia 1 year Hospice experience required with OASIS experience preferred Knowledgeable of State and Federal regulations Strong computer skills required; familiarity with Netsmart preferred Excellent observation, verbal and written communication skills, problem solving skills, basic math skills JOB
Summary:
The Registered Nurse plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience.
The professional nurse builds from resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
ESSENTIAL JOB FUNCTIONS/
Responsibilities:
Completes and initial and comprehensive assessment of patient and family to determine hospice needs.
Regularly re-evaluates patient and family/caregiver needs Develops a plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions in concert with the Interdisciplinary group Participates in on-call duties as defined by on-call policy.
Attends death visits.
Performs chart reviews, prepare interdisciplinary meeting notes.
